I understand and acknowledge that this authorization does not expire and will be in full effect for as long as I am enrolled, or otherwise participating or seeking to participate, in any resources, loans, programs and/or services (“HIRE360 Services”) provided by HIRE360 and any entities who work in partnership with HIRE360, including other workforce training service providers, government entities, and financial institutions (“HIRE360 Partners”), including any follow-up services.

 

1. Release and Indemnification. I authorize HIRE360 and HIRE360 Partners, as well as their officers, agents, employees, and contractors, to the furnish, possess, and/or discuss any information provided by me, or provided by any third party and regarding me, in connection with HIRE360 Services, including to determine if I am eligible for additional services from any party or to assist employers or apprenticeship programs in identifying and contacting me in connection with potential employment opportunities (including information provided in this application, Employment Status Information, and Program Information). I further release HIRE360 and HIRE360 Partners, as well as their officers, agents, employees, and contractors from any liability for, and waive any claims I may have relating to such activities. I shall indemnify and hold harmless HIRE360 and HIRE360 Partners, and each of their respective officers, agents, employees, and contractors, from and against any and all claims, actions, damages, suits, liabilities, obligations, costs, fees, charges, and any other expenses whatsoever, including reasonable attorneys’ fees and costs, asserted against them in connection with the furnishing, possession, or discussion of such information.

 

2. HIRE360 and HIRE360 Partners are not employers and do not make employment decisions. I understand that HIRE360 and HIRE360 Partners are not employers. I further understand that HIRE360 and HIRE360 do not participate in any employment decisions, which are made solely by contractors who choose to participate in the relevant HIRE360 Services, and that this application does not constitute an agreement or contract for employment for any specified period or designated duration.

 

3. Released Material. I understand that HIRE360 and/or HIRE 360 Partners may compile information provided by or regarding me and/or transcribe, photograph, and take audio and/or video recordings at any event which I participate or attend (together, “Released Material”). I hereby consent to and give HIRE360 and/or HIRE 360 Partners, and each of their respective officers, agents, employees, and contractors (collectively, “Releasees”), the rights in perpetuity to own, utilize, license, or alter the Released Material, in any manner the Releasees may see fit and for any purpose whatsoever. Without limiting the generality of the foregoing, I hereby authorize the Releasees and grant them the rights to utilize the Released Material in print, on the internet, and in social media in connection with advertising, publicity, and promotional purposes. Releasees may use the Released Material as they choose in their sole discretion and any right to inspect or approve the finished product or products prior to use by Releasees is hereby waived.

I further release and agree to hold harmless Releasees from any liability from any and all claims, demands, costs, legal fees or causes of action that may have exist or may hereafter exist, including for libel, defamation, invasion of privacy, right of publicity, and infringement of copyright, whether intentional or otherwise, that may arise in connection with Released Material

 

4. Authorization. By providing your phone number, you agree to receive text messages from HIRE360containing updates and information regarding the services it provides you. Message and data rates may apply. Message frequency varies. You may opt-out of receiving such messages at any time by contacting HIRE360.

 

5. Certification. I certify that all information provided by me in this application is true, complete, and accurate. I understand that any information that is found to be false, incomplete, or a misrepresentation in any respect is a sufficient basis to result in any sanction determined by HIRE360 in its sole discretion to be appropriate, including my immediate:

(a) exclusion from further participation in any HIRE 360 Services offered by HIRE360 or HIRE360 Partners; and/or

(b) discharge from any employment obtained as the result of my participation in any HIRE 360 services offered by HIRE360 or HIRE360 Partners.
